William R. Terrill, 53, said he would seek medical treatment on his own and was taken to Fitzgibbon Hospital by private vehicle, according to the report.
The accident occurred about 4 p.m. as Terrill was westbound in a 2005 Ford Escape.
His car crossed the center line, troopers said. An eastbound 2002 Mercury Grand Marquis driven by Edward L. Johnson, 57, of Gilliam swerved to avoid a collision, according to the report.
The car Terrill was driving apparently struck the car Johnson was driving in the left side, troopers said.
Both vehicles came to rest on the south side of the roadway.
The investigation continues.
