Tabitha J. Madison-Triplett, 36, was pronounced dead at the scene, according to the report.
The accident occurred about 5:45 p.m. as Madison-Triplett was traveling northbound in a 2000 Dodge Intrepid and collided head-on with a 2001 International traveling southbound.
Madison-Triplett was not wearing a safety belt, troopers said.
